New Patient Registration
Patients wishing to register with our practice should speak with our receptionists who will be happy to help them through the simple process. It's free to register. You do not need proof of address or immigration status, ID or an NHS number. If you have a preference as to which GP you receive care from please speak to a receptionist in the first instance.
The Practice boundary area covers post codes DN4 9 and DN12 1. For patients who move out of area, we recommend registration locally as soon as possible. However, we are able to continue to care for patients in certain areas, adjacent to our usual area.
The practice nurse would like to see all new patients within 28 days of registering. This helps us get to know what we can do for you and sorts out any immediate concerns.
Changes to Personal Details
Please keep us up to date with changes such as names, address or phone numbers. The form below can be downloaded and used used.
